MEDICARE

$55.10 rebate

Referral: To receive a Medicare rebate an appropriate GP referral is required.

 

Ask your GP if you qualify for a Chronic Disease Management Plan. Under this plan you are able to receive up to 5 sessions annually (10 for indigenous people).

DVA

Initial & subsequent:

$66,30 (no gap fee is applied). 

Referral: To receive DVA funding an appropriate GP referral is required.

 

You are able to receive 12 sessions annually (or within a treatment cycle)

NDIS

$137.20/hour 

Eligibility: Self -managed and plan-managed funding is accepted.

 

Duration: All sessions are 60 mins.

Note: Adapt Movement is not an NDIS registered provider therefore cannot see individuals that are NDIA agency managed.

Standard Work Cover rates 

Referral: No referral needed, however Exercise Physiology has to be stated in the intervention column on the initial-progress certificate.
